Enrollment Forms
There are numerous forms required to enroll a student. A packet must be picked up at the school during school business hours.
Enrollment Forms Include:
- New Student Registration Form
- Parent Release Form
- New Student Registration Form for Gardner Dickinson
- New Student Registration Form Public High School
- New Student Registration Form Private Schools
- Student Residency Questionnaire
- Home Language Questionnaire
- Medical Form
- School Physicals/Health Appraisal Form
In order to complete the enrollment process, you MUST bring your child's immunization record and birth certificate with you to the school.
