New York State CNA Exam
The CNA Exam is Scheduled for May 1 & 2, 2019
All applications and money must be submitted by April 5, 2019
You will need:
· Social Security number for the application
· Government issued photo ID (driver’s license) and one other form of ID that includes your date of birth for the test.
· A letter will be send to you with your test date on it- you must bring that letter on the day of the test.
· You must arrive at BOCES by 8:30 am on the day of the test and you will be staying all day so you will need to pack a lunch.
Payment:
Must accompany the application
Must be a money order or certified check
Test Fee: $115.00
