The following items are required by New York State law for a child to attend a pre-kindergarten program. They must be on file within  14 days of the start of the school year.

1. Physical exam done within 1 year of September 1st of the current school year.

2. 4 doses of DtaP or DTP (diptheria, tetanus, pertussis)

4. 3 doses of IPV or OPV (polio virus)

5. 1 dose of varicella (chicken pox)

6. 1 dose of MMR (measles, mumps, rubella)

7. 1-4 doses of HIB (haemophilus influenzae type b)
     (depending on when 1st dose was given)

8. 1-4 doses PCV (pneumococcal conjugate vaccine)
     (depending on when 1st dose was given)

9. 3 doses Hep B (hepatitis B)

10. proof of lead level screening
