The following items are required by New York State law for a child to attend grades 7-12. They must be on file within 14 days of the start of the school year.

1. Physical exam done within 1 year of September 1st for those entering 7th and 10th grade

2. 3 doses of DtaP or DTP (diptheria, tetanus, pertussis)

4. 3 doses of IPV or OPV (poliomyelitis)

5. 1 dose of varicella (chicken pox)

6. 2 doses of MMR (measles, mumps, rubella)

7. 3 doses Hep B (hepatitis B)

8. 1 dose of Tdap booster (tetanus, diptheria, pertussis)
