The following items are required by New York State law for a child to attend grade 6. They must be on file within 14 days of the start of the school year.

1. 3 doses of DtaP or DTP (diptheria, tetanus, pertussis)

2. 3-4 doses of IPV or OPV (poliomyelitis) (depending      on when 1st dose was given)

3. 2 doses of varicella (chicken pox)

4. 2 doses of MMR (measles, mumps, rubella)

5. 3 doses Hep B (hepatitis B)

6. 1 dose of Tdap booster (tetanus, diptheria, pertussis)
