The following items are required by New York State law for a child to attend grades 1-5. They must be on file within 14 days of the start of the school year.

1. Physical exam done within 1 year of September 1st of the current school year for those entering grades 2 & 4 only

2. 4-5 doses of DtaP or DTP (diptheria, tetanus, pertussis)(depending on when 1st dose was given)

4. 3 doses of IPV or OPV (polio virus)

5. 1 dose of varicella (chicken pox)

6. 2 doses of MMR (measles/mumps/rubella)(by age 7)

7. 3 doses Hep B (hepatitis B)
