The following items are required by New York State law for a child to attend a kindergarten program. They must be on file within 14 school days of the start of the school year.
1. Physical exam done within 1 year of September 1st of the current school year
2. 4-5 doses of DtaP or DTP (diptheria, tetanus, pertussis) (depending on when 1st dose was given)
4. 3-4 doses of IPV or OPV (polio virus) (depending on when 1st dose was given)
5. 2 doses of varicella (chicken pox)
6. 1 dose of MMR (measles, mumps, rubella)
9. 3 doses Hep B (hepatitis B)
10. Proof of lead level screening only if proof has not been submitted previously
